Transition in Place (TIP)

Transition in Place (TIP) – provides short-term rental subsidies and supportive services for veteran families, including single households, that are experiencing homeless or imminent risk of homelessness, as defined by the Veterans Affairs (V.A), the funding entity. TIP master-leases 20 apartments and is committed to achieving/exceeding performance measures consistent with Plan 2.0, as defined in the Program Models Chart, PH section, utilizing a scattered site model.

The “Transition in Place” program utilizes existing residential housing units where homeless veteran participants and their families receive time-limited rental assistance and supportive services intended to address underlying causes of their homelessness and build resilience and independence.

VA Case Management (VA-CM)

VA Case Management (VA-CM) facilitates services that help Veterans with retaining housing stability to prevent a return to homelessness. Each household receives time-limited supportive services intended to address underlying causes of their homelessness and build resilience and independence. Once intake is completed and a comprehensive assessment including a psycho-social assessment is conducted, an Individual Service Plan (ISP) is developed in collaboration with the participant that serves as a roadmap to independence, documenting progress and identifying barriers that may hinder maintaining permanent housing.

Honor Grant (VAAS)

Honor Grant (VAAS) -funded through the Cook County Dept. Veteran Affairs, provides continued veteran support programming in employment services, mental/physical health, housing, financial support services, and other additional social services that address the unique concerns of Cook County veterans.
